In regard to the above service address, the signature below authorizes and instructs Fins Environmental Service, LLC d/b/a Honey Wagon Septic to undertake and perform the following. *Please note: These prices are subject to change per township.*

Due to unforeseen circumstances, additional work may be required to complete the inspection. Fins Environmental Service, LLC will advise the undersigned of the estimated additional costs before proceeding.  Terms of payment are Net 30 or at time of settlement. If settlement is canceled, payment is due 30 days after service date. All late fees will apply.

 

Fins Environmental Service, LLC d/b/a Honey Wagon Septic will perform the work in a good and workmanlike manner in accordance with the current Onsite Wastewater Treatment System Inspection Standards adopted by the National Onsite Foundation.

 

If any applicable law, regulation or ordinance requires that the report be filed with or submitted to a state or local agency, or other person, the undersigned acknowledges and agrees that the undersigned property owner or agent shall make such a submittal, and if the requirement to file such report rests upon the inspector, the undersigned agrees, and acknowledges that such submittal shall be made.

 

By signing this form the property owner concedes permission to Fins Environmental Service, LLC d/b/a Honey Wagon Septic permission to enter the listed property to perform the acknowledged tasks.

 

Fins Environmental Service, LLC d/b/a Honey Wagon Septic is authorized to disclose the results of the inspection and any and all records relating to the system’s installation, repair or maintenance history to (if no one, indicate no one).

 

If we are unable to perform any of the above services upon arrival due to septic tank lid(s) not being exposed, no electric at property, no water onsite or any other issues that may arise a trip charge fee will be billed.  The property seller and/or owner will be responsible for these charges.

By signing this document as an authorized agent, you then incur the responsibility of any fees owed to Fins Environmental Service, LLC d/b/a Honey Wagon Septic if payment is not received in accordance with the terms set forth in this agreement.

No Services will be preformed until ALL paperwork is completed and returned.
